Hi Rotarians,

Please the on the attached MS Word document and return to Dean Watson and me. 

Remember two things: 

1. providing the information requested is totally voluntary. If you don't want to answer a question then you don't have to. However, for the good of the order and a better, more connected club, we encourage you to do so.

2. The information is private. No one outside Mendocino Rotary Club will receive a member directory when it is completed.

Thanks ... and don't be shy.

Personally, I think it would be nice is we knew everyone's ages as a way of gauging our clubs interests, culture, and the age of the club overall.

Again, that is your personal choice.
