Susan, thank you for notifying us of the open registration. If any one needs help with the online registration, I am happy to do the online work for you. Just let me know your needs! I will need all the same information as the online registration to input into the program. Your name, email, telephone number, name of quilt , size ( width & length) , who quilted it, and your story. And if you wish to provide your quilt to a specific merchant, the name of that merchant please!

Fill it out! Remember,you'll need to fill out one form per quilt.
