[Oceanwavequilters] Biography Questionnaire

As promised at the last meeting (and as noted in this month's newsletter that you should have received via listserv yesterday), the Biography Questionnaire is now live!



Go to oceanwavequilters.com/forms/ (or oceanwavequilters.com, click on the Forms
and Documents tab). The first item in the list is the OWQ Biography Questionnaire.
Click on that document and it will open in a new tab. Fill out the information - the only
required information is your email and your name -  the rest is to help us get to know you better! (And remember, we have several new members who DON'T know you, so please help them out by participating!!)



Complete the 
information and click the SUBMIT button at the bottom of the last page. That's it - you're
done! 



The information will automatically go into a spreadsheet in a secure file on my
computer. When I have received all of the responses, I will be organizing them into an
easy-to-read document for you. This document will only be shared within the OWQ
group - it will not be published on-line or shared with anyone outside of our group.




For those members without an email address, a hard copy of this survey is being mailed with instructions to return it via snail mail. If you know of someone who cannot access the questionnaire via computer, tell them to reach out to me and I'll get them a copy.



Please try to get your responses in by August 15th (because otherwise you'll forget... ask me how I know!)
