[Oceanwavequilters] COVID documentation

We're working with Jill Rexrode at the Senior Center to get back into our meeting room in the near future.



One of the requirements is that we maintain documentation of COVID vaccination status. Only members who have been fully vaccinated (or who have proof of a negative COVID test within the previous 72 hours) are permitted to attend gatherings at the Senior Center; this is their rule, not Ocean Wave Quilters'. It is also a rule that other senior centers in Mendocino County are following after consultation with the county health department.



To meet this requirement, I am requesting that you provide me a copy of your COVID vaccination card. You can do this in one of these ways:



Take a photo or scan of your card and email it to me.


Take a photo copy of your card and give it to me at the next OWQ meeting on July 11th, which will be outdoors at Lonna's house.


Take a photo copy of your card and mail it to me at PO Box 1653, Mendocino 95460.




MAKE SURE YOUR NAME IS ON THE CARD before you make the copy.    



I will maintain these copies in a secure file. They will be shown ONLY to Jill or her designee at the Senior Center upon request.
