Survey Monkey can be configured so the survey can only be taken once by
a particular computer. I'm not sure if it could be set up so different
people could take it from the same computer.
I've used it for a couple of surveys of a group of about 20 people and
thought that it provided me with lots of good info about the responses.
With a free account you can only have ten questions, but Norman only has
seven, and they can be in many different formats from yes/no to comments.
I'd be happy to work with you Norman to create a good Survey Monkey survey.
Anyone with the password to the account can look at the results at any time.
