I'd add JPR to the list - even if just as a statistical component.
#4 and the general tone of the survey could stand some more thought,  My 
take is that the community is in the middle of a dramatic change where 
we must understand much more about the close in communication aspects 
that govern our lives in subtle but powerful ways.  May be that the 
quality of communication defines the quality of community life - and 
likely it's evolutionary path.
I suggest that the questions be graded in a 0-9  preference fashion 
rather than yes no style to discover congruities rather than 
differences; y/n answers make one a bit crazy and do not inform for action.
This is a great way to discover  a new path to our community 
communication - but please remember, it is the questions that really
